What an MSL does
An MSL works at the intersection of medical science and external engagement. Core activities include discussing clinical evidence with physicians and other healthcare stakeholders, responding to scientific requests with objective information, building relationships with key external experts and institutions, and identifying local care-pathway or evidence gaps that can inform Medical Affairs plans.
The work can also support evidence-generation activities, such as real-world-evidence projects, local clinical collaborations, investigator-initiated studies and operational aspects of clinical trials. In India, any medicine-related promotion and information exchange must be handled within the UCPMP 2024 framework: information must be balanced, up to date, verifiable, non-misleading and aligned with marketing approval.

Entry routes in India
A practical entry route is to build a strong biomedical, medical or pharmacy foundation and then gain relevant therapeutic-area, clinical-research, Medical Affairs or field-facing pharmaceutical experience. Pharmacy is one recognised education route in India: PCI regulates pharmacy education and identifies B.Pharm, Pharm.D and M.Pharm among its approved courses. Employers’ requirements nevertheless vary by company, therapy area and seniority; an MSL role is not automatically available to every holder of one particular qualification.
For someone progressing from the parent role of Medical Representative, the transition is more credible when sales-facing experience is supplemented with deep disease-area knowledge, ability to discuss evidence rather than only product messages, compliant stakeholder engagement, and exposure to clinical research or medical functions. One employer’s MSL posting explicitly treats prior MSL, key-account-management or medical-representative experience as potentially relevant, while also asking for medicine, pharmacy or biomedical-science education and pharmaceutical-industry experience.
- Complete a relevant science, pharmacy or clinical degree; select a PCI-approved institution when pursuing a pharmacy qualification.
- Develop experience in a focused therapeutic area through clinical practice, research, pharmacovigilance, clinical operations, medical information, medical representative work or another pharmaceutical role.
- Learn to read clinical studies, interpret endpoints and communicate the limits as well as the findings of evidence.
- Target junior MSL, medical-scientific liaison, regional medical advisor or related field-medical openings; assess each vacancy’s stated degree and experience requirements.
- Treat certificates in clinical research or pharmacovigilance as supplementary evidence of preparation, not as a universal substitute for scientific expertise or role experience.
Skills to develop
Scientific credibility is the foundation: MSL employers seek the ability to explain scientific content and its context to different audiences, understand disease states and treatments, and work with clinical-trial methods, epidemiology, real-world evidence and outcomes research. An MSL also needs the judgement to present information objectively and compliantly.
The role is highly relational and operational. Strong written and spoken communication, presentations, stakeholder management, project planning, documentation and collaboration across a matrix organisation are recurring employer requirements. Because work is usually assigned by territory or account, travel readiness and effective time management are also important.

Where and how the work is performed
MSLs are typically field medical professionals rather than purely office-based staff. They cover a defined geography, set of accounts or therapy area; meet healthcare professionals and external experts; participate in external scientific activities; and coordinate with office-based Medical Affairs and cross-functional colleagues. The amount of travel varies with territory and employer.
In India, closely related field-medical vacancies may be titled Medical Science Liaison, Medical Scientific Liaison or Regional Medical Advisor. Employer listings have shown India-based regional medical roles in cities including Bengaluru, Delhi, Hyderabad, Kolkata and Mumbai, illustrating that titles and territory structures differ by organisation rather than following one nationwide naming convention.
- Territory- or account-based external scientific engagement.
- Regular coordination with Medical Affairs, clinical, regulatory, market-access and commercial-adjacent teams, with compliance safeguards.
- Travel to hospitals, institutions, advisory boards, congresses and educational meetings may be required.

Possible progression
Career progression is employer-specific, but job titles in field medical commonly show movement from MSL or equivalent field-medical work to Senior MSL and then to MSL Manager or broader Medical Affairs leadership. Progression normally requires greater therapeutic-area depth, strategic account capability, evidence-generation leadership, mentoring and ability to influence cross-functional medical strategy.
A Medical Representative background can be relevant to an entry transition, but progression in MSL work depends on demonstrating scientific credibility and compliant medical engagement. It should not be assumed that sales tenure alone meets a particular employer’s MSL criteria.
