Clinical responsibilities under senior supervision
A Junior Resident is ordinarily responsible for the patients allotted to them: examining a newly admitted patient, completing the case sheet, showing the case to a Senior Resident, ensuring investigations are obtained and recorded, and entering daily follow-up notes. In outpatient care, the Junior Resident may present a concise history and physical findings to a Senior Resident.
For acutely ill patients and critical investigation results, the role requires prompt escalation to the Senior Resident, GDMO or specialist and following the senior clinician’s directions. This makes the post a supervised service-and-training role, not independent specialist practice.

- Coordinate and document laboratory and imaging investigations, including urgent results.

Entry route after MBBS
The usual route is: complete an MBBS degree, complete the compulsory rotating medical internship, obtain the registration required by the recruiting jurisdiction, and apply to hospital Junior Resident vacancies. The NMC’s CRMI guidance says all medical graduates must undergo the internship to be eligible for permanent registration to practise medicine in India, and specifies a minimum 12-month internship after the final MBBS examination.
Employer notices determine the operative eligibility, documents, recency conditions and selection method. For example, VMMC & Safdarjung’s 2025 regular Junior Resident notice required a recognised MBBS degree, internship completed on or after July 1, 2023, DMC registration (with original registration required before joining), and used a written screening test followed by counselling. These conditions should not be treated as universal rules for every Indian hospital.

These are evidenced by the hospital duties assigned to Junior Residents: record a history and physical findings, complete case sheets and follow-up entries, arrange and record investigations, communicate critical reports to senior colleagues, and immediately seek advice for acutely ill patients.
Because duties are rostered across wards and emergency settings and care is overseen by Senior Residents and specialists, the role also requires reliable handover, teamwork, responsiveness to supervision and disciplined adherence to departmental instructions.

Hospital-based, rostered work
Junior Residents work in hospital clinical units rather than a single fixed setting. The cited central-government hospital manual places them in wards, outpatient departments and the Emergency Department, with duty also scheduled at night, on Sundays and gazetted holidays according to roster.
The workload and exact rotation structure vary by hospital and department. One 2025 VMMC & Safdarjung appointment specified a one-year tenure with two six-month subject rotations, illustrating how hospitals may combine service delivery with exposure across departments.

- OPD: initial assessment and referral/presentation to Senior Residents when needed.

Possible next steps
Junior residency can provide hospital experience before postgraduate training, but it is not itself a postgraduate qualification. For admission to a postgraduate medical course, NMC’s published postgraduate regulations require a recognised MBBS degree or equivalent qualification and permanent registration with the Medical Council of India or a State Medical Council, or registration within one month of admission.
A subsequent Senior Resident route is institution-specific. At VMMC & Safdarjung, the published eligibility for Senior Resident posts is a recognised postgraduate degree/diploma in the concerned specialty; where such candidates are unavailable, the institution lists MBBS plus two years’ government-organisation experience, including one year in the concerned specialty. Candidates should therefore distinguish between progressing through postgraduate entrance/admission and meeting a particular employer’s Senior Resident recruitment conditions.

Compensation: use current employer notices
There is no single national Junior Resident salary that safely represents all Indian employers. Pay differs across central and state government hospitals, autonomous institutions, medical colleges and private employers, and the take-home amount is affected by appointment terms and allowances.
As a dated public-sector example, VMMC & Safdarjung Hospital’s February 18, 2026 notice reported monthly Junior Resident basic pay of ₹56,100, NPA of ₹11,220, HRA of ₹16,830, DA of ₹39,046 and TA of ₹11,376, for a stated total of ₹134,572. Its May 2025 recruitment notice also described the post as Pay Level 10, ₹56,100 plus NPA and other admissible allowances. These figures are local and time-bound; applicants should consult the newest notice and applicable service rules before relying on them.
- Check whether the post is regular, ad hoc, contractual or tenure-based.
- Separate basic pay from NPA and location-dependent allowances.
- Confirm whether the published figure is gross monthly pay, consolidated remuneration or expected take-home pay.
- Treat all salary figures as dated institutional context, not a national benchmark.
